At Summerhill Health, we provide thorough, in-person testosterone replacement therapy in a supportive, unhurried environment. From your first blood test through to long-term monitoring, you’ll be cared for by Dr Summerhill from start to finish. Your journey with us includes:

Step 1: Your morning blood test

Testosterone levels follow a daily rhythm and are highest in the early morning, which means your first test must be taken between 7am and 11am. If you work nights or shifts, please email us for personalised advice, as the test needs to be taken at the start of your day. A test taken later can read artificially low and lead to a misdiagnosis.

This is a formal venous blood draw (not a finger-prick test). You’ll need to avoid strenuous exercise and ejaculation for 48 hours before the test.

We check five specific markers: Testosterone, Sex Hormone Binding Globulin (SHBG), Free Androgen Index, Free Testosterone, and Albumin. You’ll also complete a symptom questionnaire, including the ADAM and ILET assessment tools, so we can build a full picture alongside the blood results.

We consider total testosterone low when it falls:

  • Below 12 nmol/L in most men
  • Below 14 nmol/L if you’re living with diabetes or pre-diabetes (these conditions can alter how testosterone behaves in the body)

Borderline results (around 15 to 16 nmol/L) are also flagged for further investigation. If your first result is in range, we’ll review the wider picture and suggest next steps. If it’s low or borderline, we move to step 2.

Step 2: Repeat and extended bloods (around four weeks later)

Testosterone fluctuates with sleep, stress, recent illness, and even the time of year. UK and international medical guidance is to confirm a low result with a second morning test before starting treatment. At the same appointment, we run a wider panel to investigate the cause of the deficiency, which is just as important as treating it.

The extended panel typically includes:

  • LH and FSH (pituitary function)
  • SHBG and free testosterone
  • Prolactin
  • Thyroid function
  • Ferritin and full blood count
  • HbA1c and lipid profile
  • Kidney and liver function
  • PSA, where appropriate

This second round of tests tells us whether the cause is primary (the testes), secondary (the pituitary), or related to lifestyle, weight, sleep, or another underlying condition. The answer shapes the right treatment plan.

Step 3: Your initial consultation with Dr Summerhill

Once your repeat blood test results are back, you’ll book a 60-minute initial consultation with Dr Summerhill. This is where we:

  • Talk through your results in plain language
  • Review your symptoms, medical history, and lifestyle
  • Discuss whether TRT is the right step for you, and which formulation fits you best
  • Issue your prescription
  • Show you how to administer your medication
  • Provide your first dose in clinic, so you leave with treatment underway and your questions answered

You’ll also leave with a clear monitoring plan and your follow-up schedule.

Step 4: Ongoing reviews and monitoring

TRT is usually a long-term commitment, and safe treatment depends on regular review. Your first review takes place at 2 to 3 months, with trough blood tests (19 markers) timed to capture your lowest hormone level: at least 6 hours after gel application, or just before your next injection is due. The same pre-test rules apply (morning draw, no strenuous exercise or ejaculation within 48 hours).

During the first year, expect appointments every 2 to 6 months as we fine-tune your dose. Once you’re established on the right level, reviews typically move to every 6 to 12 months. You can also add an annual health screening alongside your monitoring appointments if you wish.

We monitor testosterone, haematocrit (red blood cell concentration), PSA, lipids, and liver function to make sure treatment stays both effective and safe.

Treatment options we offer

We offer all three main forms of testosterone replacement, and the right one for you depends on your routine, preferences, and clinical picture.

  • Testosterone gel (e.g. Testogel): applied to the skin daily for steady levels. Suits men who prefer a daily routine and no needles.
  • Oral testosterone (tablets): a newer option for some men, taken with food.
  • Testosterone injections (e.g. testosterone cypionate): intramuscular injections every one to four weeks. Suits men who prefer less frequent dosing.

  • Low testosterone rarely shows up as a single dramatic symptom. It’s usually a cluster of changes that creep in over months or years. The most common signs include persistent fatigue, low libido or erectile difficulties, loss of muscle and an increase in body fat (particularly around the waist), low mood, irritability, or brain fog, and disturbed sleep. If you recognise three or four of these and they’ve been going on for several months, it’s worth investigating with a morning blood test.

  • No. You can book your first morning testosterone blood test directly. Once your results are in, you’ll book a 60-minute initial consultation with Dr Summerhill to review them, discuss your treatment options, and start TRT in clinic if it’s appropriate.

  • Often, yes. We’ll need a recent morning testosterone result alongside the wider markers we use to interpret it: LH, FSH, SHBG, free testosterone, prolactin, thyroid function, ferritin, full blood count, HbA1c, lipid profile, and PSA where appropriate.

    If your existing tests don’t cover the full panel, we’ll let you know what’s missing before your initial consultation, so you can either top up or book a new test with us.

  • We prescribe all three main forms of testosterone replacement:

    • Acne or oily skin, particularly in the early months
    • Fluid retention in some men
    • Reduced sperm production and fertility (see the fertility FAQ below)
    • Tenderness or enlargement of breast tissue in a small number of cases
    • Mood changes, which usually settle as the dose is fine-tuned

    We monitor testosterone, haematocrit, PSA, lipids, and liver function on a defined schedule to catch any changes early and keep treatment both effective and safe. TRT is not appropriate for men with active prostate or breast cancer, and we’ll discuss any other medical history that might affect your suitability at your consultation.

  • Our pricing is built around three components:

    • blood tests
    • your initial consultation with Dr Summerhill
    • and your ongoing medication and reviews.

    Each is priced separately, so you only ever pay for what you need. We’ll always be clear about costs before you proceed, with no hidden fees.

  • Online clinics typically offer flat-fee monthly subscriptions, which can look cheaper at first glance. The trade-offs are the depth of assessment (often a single finger-prick test and a short questionnaire) and the breadth of monitoring.

    Subscriptions can also exclude the consultations, repeat bloods, and additional therapies you may genuinely need. We price each element separately, so you’re paying for what’s clinically appropriate for you, in person, with continuity of care.

  • Improvements in energy, mood, and libido often begin within the first 4 to 6 weeks. Changes in body composition (more muscle, less fat) tend to develop over 3 to 6 months. We’ll review the results with you to check your response and adjust the plan if needed.

  • Yes. Testosterone replacement significantly reduces sperm production and is not appropriate if you’re trying to conceive now or in the near future. If fertility matters to you, we’ll discuss alternative approaches at your initial consultation.

  • In many cases, yes. We’ll need your recent blood results, current prescription details, and a brief overview of your regimen. You’ll still need an initial consultation so we can take a full history and agree on the ongoing monitoring plan, but in most cases, we can continue treatment without a break.

  • While we offer video consultations and home visits for some of our other services, TRT is delivered in person. Your blood draw, initial consultation, and first dose all take place at our clinic in Hale.

    A face-to-face assessment lets Dr Summerhill take a full history, examine you, and walk you through how to administer your medication safely. It also means your bloods are drawn under the right conditions rather than relying on a finger-prick kit posted home.

    Once you’re established on treatment, brief follow-up reviews can sometimes be handled remotely, but the foundation of TRT care at Summerhill Health is face-to-face.

  • Yes. We see men from across Greater Manchester, Cheshire, and the wider North West. Same-week and evening appointments are usually available, so TRT can fit around any work and family commitments without weeks of waiting.

  • If you choose to stop treatment or if Dr Summerhill recommends it, we’ll guide you through it safely. A review or follow-up bloods may be advised to monitor how your hormone levels respond. You’ll only be charged for the medication and care provided up to that point.
