Adjusting Your HRT Dose? What To Expect
“I’ve been on HRT for three months, but I’m still having hot flushes.”
“My energy’s better, but the brain fog hasn’t shifted.”
“How long should I wait before asking about a dose change?”
Do any of these thoughts sound familiar? Finding the right HRT dose is rarely a one-step process. At our private GP clinic in Hale near Manchester, Dr Natalie Summerhill provides expert menopause care that recognises this reality, supporting women across Manchester and Cheshire through the adjustment process with personalised, thorough consultations.
As a British Menopause Society-certified specialist, Dr Summerhill brings extensive experience in hormone optimisation. Her approach combines clinical expertise with a genuine understanding of how menopause symptoms affect your work, relationships and day-to-day.
Our evidence-based approach means careful monitoring and thoughtful adjustments based on both your symptoms and how you respond to treatment. Whether you’re starting HRT for the first time, fine-tuning an existing prescription, or managing persistent symptoms, we provide comprehensive support with longer appointment times that give you space to discuss every concern.
With flexible consultation options available both in-person and via video, accessing specialist menopause care is straightforward. Our same-day and evening appointments mean you don’t have to wait weeks between adjustments, allowing us to optimise your treatment efficiently.
Understanding HRT Dose Adjustments
Hormone Replacement Therapy (HRT) replaces the oestrogen and, when needed, progesterone that your body stops producing during menopause. Finding the right dose is a careful balancing act. Too little, and symptoms persist. Too much, and you may experience side effects like breast tenderness or bloating.
The adjustment process is about systematic optimisation based on your individual response, symptom patterns, and how you metabolise hormones. This is where specialist expertise makes a real difference.
Why Initial Doses May Need Adjusting
Many women start on a standard or lower dose of HRT. This cautious approach allows your body to adjust gradually and helps identify the minimum effective dose for you. However, what works initially may not provide complete symptom relief, or your needs may change over time.
Common reasons for dose adjustment include:
- Persistent symptoms despite several months on treatment
- Partial improvement where some symptoms resolve but others remain
- Symptoms return after initial relief
- Life changes affecting hormone needs (stress, weight change, other medications)
- Side effects suggesting the dose is too high
- Perimenopause progresses to full menopause, requiring a different hormone balance

New patients must book an initial consultation. This isn’t required for contraception services or blood tests.
A review of my hormone levels, HRT and blood pressure issues.
I had my first consultation with Dr Summerhill this week for a Health screening, including a review of my hormone levels, HRT and blood pressure issues. It is such a relief when you can spend time with someone to discuss all aspects of your health and things of concern, without feeling like you are a nuisance. The experience was very professional, but with the care and thoughfulness for you as an individual that we need when visiting a Dr, I was very impressed and will definitely recomend.

At Summerhill Health, we provide structured, supportive private healthcare throughout the HRT optimisation process. Your journey with us includes:
Initial Assessment
- Review of your current HRT regimen and how long you’ve been taking it
- Detailed discussion of which symptoms have improved and which persist
- Evaluation of any side effects or concerns
- Assessment of your individual metabolism and response patterns
- Clear explanation of adjustment options
- Development of a personalised optimisation plan
The Adjustment Timeline
- Weeks 1-4: You’ll start your adjusted dose. Most women notice initial changes within the first two weeks, though full effects take longer. Keep note of any symptom changes or new side effects during this period.
- Weeks 4-12: This is the key assessment window. Oestrogen levels stabilise, and you’ll get a clearer picture of whether the adjustment is working. Some symptoms, particularly psychological ones like anxiety or low mood, may take the full 12 weeks to resolve.
- 3-Month Review: We’ll discuss your response to the adjustment. If symptoms have resolved, excellent. If not, we’ll consider further optimisation. For some women, finding the optimal dose requires several adjustments.
Ongoing Optimisation
- Regular progress monitoring with flexible appointment scheduling
- Quick access to advice between appointments if needed
- Systematic approach to further adjustments if required
- Long-term management plans as your needs evolve
- Coordination with other aspects of your women’s healthcare
Common Adjustment Scenarios We Support
- Increasing oestrogen dose for persistent hot flushes, night sweats, or brain fog
- Adjusting progesterone type or dose for breakthrough bleeding or mood effects
- Adding testosterone for low libido.
Please note: While some women also find testosterone helpful for energy and mental clarity, current evidence primarily supports its role in improving libido. Dr Summerhill will discuss what you might realistically expect based on your symptoms.
- Changing HRT delivery method (patch to gel, pill to patch, etc.)
- Managing side effects while maintaining symptom control
- Optimising doses during perimenopause
- Adjusting HRT alongside other treatments or medications
- Managing HRT in women with neurodiversity, where symptoms may present differently
It was great to understand how all this fits in with perimenopause.
Absolutely amazing service. As a female that is ‘too young’ for help from the NHS (despite a supportive GP) without conclusive blood tests, I was encouraged to seek the help of an expert in the field. I am so grateful I had my time with Dr Summerhill, I already feel so much better and understand so much more about my body. I was experiencing so many symptoms and it was great to discuss this with a professional and understand how all this fits in with peri-menopause. I have no hesitation in recommending Summerhill Health.
If you are reading this review wondering if you should book that appointment go ahead and do it, you will not regret it!
FAQs
The general guidance is to wait at least 8-12 weeks before making dose adjustments. This allows time for your hormone levels to stabilise and for you to experience the full effects of your current dose. If you’re experiencing significant side effects, we can review sooner.
Some symptoms respond more quickly than others:
- Hot flushes and night sweats often improve within 2-4 weeks
- Sleep quality typically improves within 4-6 weeks
- Mood, anxiety and brain fog may take 8-12 weeks to fully resolve
- Joint pain and muscle aches can take 2-3 months to improve
- Libido and energy levels may require 3-6 months to fully respond
If you’ve given your current dose adequate time and symptoms persist, it’s worth discussing an adjustment.
Several indicators suggest your HRT dose may be too low:
Persistent vasomotor symptoms:
- Hot flushes continuing at the same frequency or intensity
- Night sweats disrupting your sleep regularly
- Feeling overheated frequently
Ongoing mood and cognitive symptoms:
- Brain fog or difficulty concentrating
- Low mood or heightened anxiety
- Irritability or mood swings
- Feeling unlike yourself
Physical symptoms:
- Persistent fatigue despite good sleep
- Vaginal dryness or discomfort during intercourse
- Joint pain or muscle aches
- Ongoing sleep disruption
Timing patterns:
- Symptoms return towards the end of your patch-wearing period
- Good days and bad days with no clear pattern
- Initial improvement that has plateaued
During your consultation, we’ll assess whether these symptoms indicate under-treatment or whether something else might be contributing.
Side effects often indicate your dose needs adjustment or your HRT type needs changing. Common side effects and their usual meaning:
Breast tenderness or swelling:
- Often settles within 2-3 months as your body adjusts
- If persistent, it may indicate the oestrogen dose is too high
- Can sometimes be managed by switching from patches to gel
Bloating or fluid retention:
- Usually temporary in the first few weeks
- If ongoing, may suggest progesterone type needs changing
- Sometimes related to oestrogen dose
Headaches:
- Common in early weeks as hormone levels stabilise
- Pattern and timing help us understand the cause
- May indicate dose adjustment or delivery method change needed
Nausea:
- Usually, only with oral oestrogen tablets
- Often resolves as the body adjusts
- Switching to transdermal (patch or gel) usually eliminates this
Breakthrough bleeding:
- Common in the first 3-6 months, especially with continuous HRT
- The pattern helps us determine if a progesterone adjustment needed
- Persistent bleeding after 6 months requires investigation
Never simply stop your HRT if you experience side effects. Contact us to discuss the best approach, which might involve dose reduction, changing HRT type, or allowing more adjustment time.
No, HRT doses should only be adjusted under medical supervision. Whilst HRT is generally very safe, inappropriate dosing can cause side effects or fail to adequately protect your endometrium (womb lining) if you still have your womb.
However, you are an essential partner in the process. Our patients find that keeping a symptom diary helps enormously:
- Track which symptoms persist and their severity
- Note timing patterns (time of day, point in your patch cycle)
- Record any side effects and when they occur
- Monitor your mood, energy, and sleep quality
- Note any other changes in your health or lifestyle
This information guides our adjustment decisions and helps us optimise your treatment efficiently.
This varies significantly between women. Some find their optimal dose within one adjustment, whilst others require several changes to get it right.
Common factors affecting this include:
- How you metabolise hormones (individual variation is significant)
- Whether you’re in perimenopause or post-menopause
- Your baseline symptom severity
- Presence of other health conditions
- Other medications affecting hormone levels
- Your individual treatment goals
The good news is that most women find their optimal dose within 2-3 adjustments, typically over 6-9 months. Our flexible appointment system means you’re not waiting months between reviews, allowing us to optimise your treatment efficiently.
Understanding this helps you describe your symptoms accurately during consultations:
Too little oestrogen:
- Hot flushes and night sweats persist
- Low mood, anxiety, or irritability continues
- Brain fog or memory issues
- Ongoing fatigue
- Vaginal dryness
- Joint pains
- Sleep problems
Too much oestrogen:
- Breast tenderness or swelling
- Bloating or fluid retention
- Nausea (with oral tablets)
- Headaches (sometimes)
- Leg cramps
- Generally feeling “too oestrogenised”
Most women find that they feel best on the lowest dose that fully controls their symptoms. This optimal dose sits in what we call the “therapeutic window”; enough to eliminate symptoms without causing side effects.
Possibly. Several factors might prompt dose changes after you’ve found what works:
In perimenopause:
- Your own oestrogen production fluctuates, so HRT needs may vary
- Symptoms may change as you progress towards full menopause
- Adjustments may be needed every 6-12 months
Post-menopause:
- Once you’re several years past your final period, your dose often stabilises
- Some women find they need slightly less oestrogen as they get older
- Changes in weight, stress, or other health conditions may affect needs
Lifestyle factors:
- Significant stress can affect hormone requirements
- Weight changes alter hormone metabolism
- New medications may interact with HRT
- Health conditions may influence dosing decisions
We recommend annual reviews even when your HRT is working well. This allows us to ensure your treatment continues to meet your needs and remains appropriate for your health status.
Body-identical HRT (micronised progesterone and oestradiol) is chemically identical to the hormones your body produces naturally. We strongly prefer these preparations at Summerhill Health, and Dr Summerhill can explain the evidence behind this during your consultation.
For dosing purposes, body-identical oestradiol comes in various strengths, whether it’s a patch, gel, or spray. The principles of dose adjustment remain the same. The advantage of body-identical preparations is that they appear to carry lower risks, particularly regarding breast cancer and blood clots, compared to older synthetic formulations.
If you’re currently on older-style HRT and considering switching, we can guide you through this transition whilst ensuring continuous symptom control.
Contact us promptly if you experience:
- Sudden severe headaches or visual changes
- Chest pain or unexplained breathlessness
- Leg pain with swelling or redness
- Heavy vaginal bleeding or unusual bleeding patterns
- Severe abdominal pain
- New breast lumps or significant breast changes
These are rare but require immediate assessment.
