Summerhill Health – Signature Care Membership Terms & Conditions
Welcome to Summerhill Health (“the Practice”). By activating your Signature Care Membership (“the Membership”), you (“the Member” or “Patient”) agree to be bound by these Terms and Conditions. Please read them carefully.
1. Scope of Membership Benefits Your Membership includes the following specific benefits, subject to availability and the conditions outlined below: GP Consultations: Up to 30 GP appointments per membership year. This allocation is subject to our Fair Usage Policy (Section 4). All appointments are conducted within standard clinic hours (8:00am to 6:00pm). Priority Appointments: Same-day appointments will be provided within clinic hours wherever clinically and operationally possible. Dedicated WhatsApp Line: Members receive exclusive access to a dedicated WhatsApp communication line to submit appointment enquiries, request rescheduling, and message the administration team during clinic hours. Fast-Track Referrals & Repeat Prescriptions: Administrative processing for private specialist referrals and repeat prescription requests is included. Note: The cost of the actual medication and third-party specialist fees are not covered by the Practice. Continuity of Care: We will endeavor to book you with your dedicated GP wherever possible, subject to their working hours and leave. Annual Health Review: One comprehensive health review is included per 12-month cycle. Preferential Rates: Members receive a 10% discount on in-house lab tests, diagnostics, and other in-house procedures, and a 20% discount on home visits.
2. Contract Term, Fees, and Billing 12-Month Commitment: The Membership is a fixed 12-month binding agreement from the date of activation. Payment Options: Payments can be made via a single upfront annual payment or in 12 consecutive monthly installments. Automatic Renewal: To ensure uninterrupted care, your membership will automatically renew for a further 12 months at the end of your term unless you notify us of your intent to cancel at least 30 days prior to your renewal date. Late/Failed Payments: If a monthly installment fails, the Practice reserves the right to suspend your membership benefits, including appointment booking, until the balance is cleared.
3. Cancellation and Refund Policy Cooling-Off Period: You have the right to cancel your membership within 14 days of activation. If you have used any services (including an appointment or health review) during this time, you will be charged the standard non-member rate for those services before a refund is issued. Early Termination: Because this is a 12-month agreement, cancellation prior to the end of the 12th month is not permitted except in exceptional circumstances (e.g., relocation outside our service area) at the sole discretion of the Practice management.
4. Fair Usage, Booking, and Attendance Fair Usage Policy: The allowance of up to 30 appointments per year is intended to manage genuine health needs. To ensure all members have equitable access to our clinicians, appointments must be spaced reasonably. Grouping or block-booking excessive appointments in a short timeframe may prompt a clinical review of your membership. Cancellations & No-Shows: If you need to cancel or reschedule an appointment, you must give at least 24 hours’ notice. Failure to do so, or failing to attend a scheduled appointment, will result in that appointment being deducted from your annual 30-appointment allowance.
5. Service Limitations and Emergency Care: Summerhill Health provides scheduled private primary care during clinic hours (8:00am to 6:00pm). We do not provide 24/7 accident and emergency medical services, and our WhatsApp line is not monitored outside of clinic hours or for medical emergencies. In the event of a medical emergency, you must dial 999 or go to your nearest NHS Accident & Emergency (A&E) department.
6. Termination by the Practice The Practice reserves the right to terminate your membership immediately and without a refund if: Membership fees remain unpaid for more than 14 days. A Member exhibits abusive, aggressive, or inappropriate behavior toward any doctor, staff member, or other patient at Summerhill Health.
7. Governing Law These Terms and Conditions are governed by the laws of England and Wales, and any disputes will be subject to the exclusive jurisdiction of the courts of England and Wales.
